Abstract

The purpose of the study – morphological analysis and analysis of insulin-like growth factor 1 and 2 receptor expression (rIGF-1 and rIGF-2, respectively) in samples of adrenal tissue removed during surgery in patients with benign neoplasms of adrenal glands, depending on body mass index (BMI).

Materials and methods. An analysis of the adrenal glands removed during surgery was carried out. The nosologies studied were focal nodular hyperplasia and benign neoplasms of adrenal glands. The material of 37 observations was divided into three groups: group 1 (comparison group) (n=9) – patients with BMI<25 kg/m2, group 2 (n=15) – patients with a BMI of 25-30 kg/m2 and group 3 (n=13) – patients with a BMI >30 kg/m2. Micropreparations were used for a general assessment of the state of tissues, microscopic examination, and morphometric examination of adrenal glands. The expression of IGF-1 and rIGF-2 in adrenal glands tissue was analyzed using primary monoclonal antibodies from DAKO (Denmark), TermoScientific (Germany), and Diagnostic BioSystems (USA).

Results and conclusions. Immunohistochemical analysis of adrenal gland tissue in patients with overweight and obesity revealed moderate (2++) and distinct (3+++) expression of rIGF-1 with weak expression in the group with normal body weight. The presence of a strong correlation between the severity of expression of rIGF-1 and BMI in groups of patients with overweight and obesity has been established. The expressiveness of the expression of IGF-1 has a clear unidirectional growth, which demonstrates the trend line. In patients with normal body weight, rIGF-2 expression was not observed. It has been established that with the growth of BMI, there is a tendency to reduce the expression of rIGF-2. Focal expression of rIGF-2 in adrenal glands was found only in obese patients. The presence of a strong correlation between the severity of IGF-1 expression in adrenal gland tissue and BMI tissue samples has been established, which may indicate the participation of IGF-1 in the pathogenesis of focal nodular hyperplasia and benign neoplasms of adrenal glands. The adult population of Ukraine with overweight and obese can be considered a risk group for the development of focal nodular hyperplasia and benign neoplasms of adrenal glands, which requires the development of appropriate screening programs.
